Linfoma angioinmunoblástico de células T: informe de un caso que demuestra la utilidad del análisis mutacional molecular

El linfoma angioinmunoblástico de células T (AITL) es un linfoma periférico de células T distintivo, aunque poco frecuente. En Estados Unidos, la incidencia es de aproximadamente 0,05 casos por 100 000 personas-año, lo que representa entre el 15 % y el 20 % de los linfomas periféricos de células T y entre el 1 % y el 2 % de los linfomas no Hodgkin. Se describió el caso de un hombre de 62 años con síntomas sistémicos y linfadenopatía difusa. Las células neoplásicas carecían de CD10/BCL6, pero mostraban una fuerte expresión de PD-1, ICOS y CXCL13, y albergaban mutaciones en TET2 y DNMT3A, lo que confirmó un fenotipo de células T cooperadoras foliculares (TFH). La inmunotinción expandida de TFH, combinada con el análisis mutacional dirigido, puede resolver las presentaciones de AITL con diagnóstico ambiguo.
