Abstract

A descriptive study was carried out with the use of qualitative and quantitative methods, with the objective of elaborating a proposal of actions aimed at improving the functioning of the monitoring of the development of the child at risk, graduated from the neonatal intensive care unit of the General Hospital " Dr. Agostinho Neto "at the level of primary health care (PHC) in Guantanamo municipality. Participating in the study are 40 children graduated from this service from February 2009 to February 2010, their parents and 86 PHC professionals linked to the surveillance of children at risk of disability. Factors that affect the quality of care are identified, among others: the need for training, lack of coordination between the primary and secondary level of the health system and organizational problems. In the opinion of the experts, the proposal is an effective alternative to respond to identified problems and its generalization is feasible.
