Clinical, tomographic and histopathological correlation of cervical lymph nodes in patients with laryngeal carcinoma

Abstract

Introduction: laryngeal carcinoma is one of the most frequent malignancies of the head and neck, with high morbidity and mortality. The correct evaluation of cervical lymphadenopathy is essential for staging, prognosis, and therapeutic planning.

Objective: to determine the clinical, tomographic, and histopathological correlation of cervical lymph nodes in patients with laryngeal carcinoma.

Method: an observational, descriptive, and retrospective study was conducted in 26 patients with a histological diagnosis of laryngeal carcinoma, treated at the Hospital General Docente Dr. Agostinho Neto in Guantánamo between 2021 and 2023. Demographic variables, tumor location, and clinical, tomographic, and histopathological findings were analyzed. Primary data were obtained from medical record review. Descriptive statistics and frequency distributions were used to determine absolute and relative frequencies. Concordance was assessed using the Kappa coefficient.

Results: male patients predominated (88.4%), and the 60–69 age group was the most frequent. Transglottic location was the most common (61.5%). Clinical examination did not detect lymphadenopathy in 65.3% of cases, whereas tomography evidenced nodes in 95.3%. Clinical-tomographic concordance was moderate (κ=0.503; p=0.001), and tomographic-histopathological concordance was high (κ=0.810; p=0.000).

Conclusions: computed tomography demonstrates high concordance with histopathology in the evaluation of cervical lymph nodes in laryngeal carcinoma, constituting a reliable diagnostic tool for the staging of these patients.
