Management of peptic ulcer in chronic kidney disease

Introduction: peptic ulcer is a chronic gastrointestinal condition that has a higher incidence and distinctive characteristics in patients with chronic kidney disease.
Objective: to analyze the published information regarding the management of peptic ulcer in patients with chronic kidney disease and its clinical course.
Method: a bibliographic review was carried out on the management of peptic ulcer in patients with chronic kidney disease. A detailed search on the topic was used in electronic databases such as PubMed, Scopus, Web of Science and EMBASE, Google Scholar, Trip Medical Database and British Medical Journal (BMJ), published in the last ten years, available in English or Spanish. For the analysis, it resulted in a total of 30 articles that went to primary review, the final review was carried out on 10 articles. Inclusion and exclusion criteria were taken into account.
Results: the damage caused by pressure ulcer results from the breakdown of the balance that exists in healthy people and patients with chronic kidney disease, between the aggressive and defensive components of the gastroduodenal mucosa. Among the defensive factors are: mucobicarbonate barrier, mucosal blood flow, prostaglandins and cell renewal. Among the aggressive factors of the mucosa: along with hydrochloric acid, pepsin and bile reflux, of greatest interest are Helicobacter pylori infection and the consumption of non-steroidal anti-inflammatory drugs.
Conclusions: early detection of pressure ulcer in patients with chronic kidney disease is essential to avoid serious complications.
